This is caused by having too many polygons visible.
Several things could be happening:

1) You are not running vis.
2) an error in your map is causing the .prt file not to be created.
3) You have too many polygons visible.

To fix these things:

1) run vis. If this doesn't work, or you are already running vis, then...
2) Find any leaks. if there are none, look for entities outside of the
level. Move them into the level.
3) Reduce the complexity of the area.
