  I believe I can answer PART of your question.  I came across a similar
discovery when I was trying to make a room with lava big enough to hold
Shubb Niggareth.  I "accidentally" put the lava just a little above the
floor, so when i walked into it, my feet DID NOT trigger the lava to
hurt me!  It was like wading in the Lake of Fire!  hehehe... It was
quite an interesting visual trickery, one that I hadn't intended.

Anyway, my point here is that LAVA seems to be triggered by the player's
feet.  Then again, my lava pool wasn't more than waste high.  Maybe the
player just has balls of steel and it doesn't hurt him in that area?
*grin*

As far as the lava disappearing, that might have to do with what level
you VISed in.
