Don't know about a guide but...

You've got to have at LEAST 1 info_player_deathmatch entity in there as
well.  Plus I think you might need an info_player_team1 and
info_player_team2 (these are where the players come in initially).  And
the flags are item_flag_team1 and item_flag_team2.  Team1 is red and
Team2 is blue.
